Please contact us if you have any questions. We are honored that you chose us to help you meet your endodontic needs.
After you receive your endodontic treatment, Dr. Tao will personally call you to follow up. The doctor can also be reached anytime after hours following your procedure by calling her personal mobile phone number, which you are given before leaving the office.
When You Call For an Appointment
Please have ready all pertinent medical, dental and insurance information available. Our friendly staff will assist you in verifying your insurance benefits. This will save time during your initial visit and allows us to offer most treatments the same day.
Your Initial Visit
Your initial visit will consist of a consultation explaining your diagnosis and treatment options.
Please assist us by providing the following information at the time of your consultation:
- A list of medications you are presently taking
- If possible, please download, print and fill out the Patient Registration Form and Notice of Privacy Practices and bring them with you for your visit. This will save you time in our office.
Important
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the initial visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
X-Rays
If your dentist or physician has taken x-rays, you may bring them with you to our office. If additional films are necessary, they can be taken at our facility.
